[FONT="]Abstract[/FONT]
[FONT="]Background[/FONT]
[FONT="]Some studies have suggested a protective effect of antioxidant nutrients on lung cancer. Observational epidemiological studies suggest an association between higher dietary levels of fruits and vegetables containing beta carotene and a lower risk of lung cancer.[/FONT]
[FONT="]Objectives[/FONT]
[FONT="]To determine whether vitamins, minerals and other potential agents, alone or in combination, reduce incidence and mortality from lung cancer in healthy people.[/FONT]
[FONT="]Search strategy[/FONT]
[FONT="]The electronic databases MEDLINE (1966-july 2001), EMBASE (1974-july 2001) and the Cochrane Controlled Trial Register (CENTRAL, Issue 3/2001) and bibliographies were searched. In addition authors of included studies were contacted to identify potentially eligible published and unpublished trials.[/FONT]
[FONT="]Selection criteria[/FONT]
[FONT="]Included studies were randomised controlled clinical trials comparing different supplements or comparing supplements with placebo, administered to healthy people with the aim of preventing lung cancer.[/FONT]
[FONT="]Main results[/FONT]
[FONT="]Four studies were eligible for inclusion. All were population based trials, including a total of 109,394 participants. Two studies included smokers, one included workers exposed to asbestos and two studies were carried out in health professionals. A group of participants with no known risk factors for lung cancer was included in the study sample of two trials. Beta-carotene was evaluated in all trials, alone or combination with alpha-tocopherol or retinol, and one study tested alpha-tocopherol alone. Duration of treatment varied from 2 to 12 years and follow-up was from two to five years. All trials had a placebo group.
For people with risk factors for lung cancer no reduction in lung cancer incidence or mortality was found in those taking vitamins alone compared with placebo (incidence of lung cancer: RR 0.98, 95% CI 0.81-1.19; lung cancer mortality: RR 0.93, 95% CI 0.73-1.19). For people with no known risk factors of lung cancer, none of the vitamins or their combinations appeared to have any effect. Combined data from three studies showed a non-statistically significant increased risk of lung cancer incidence (RR 1.11, 95% CI 0.94-1.33) and mortality (RR 1.05, 95% CI 0.87-1.28) for beta-carotene alone at pharmacological doses in groups with risk factors for lung cancer. When beta-carotene was combined with retinol, data from a single study showed that there was a statistically significant, increased risk of lung cancer incidence (RR 1.42, 95% CI 1.13-1.80) and mortality (RR 1.75, 95% CI 1.29-2.38) in people with risk factors for lung cancer who took both vitamins compared with those who took placebo. Data from also from one study showed that the combination of beta-carotene with alpha-tocopherol in people with risk factors for lung cancer was associated with a non-statistically significant increased risk of lung cancer incidence (RR 1.16, 95% CI 0.96-1.39) and mortality (RR 1.15, 95% CI 0.91-1.45). No effect was observed for total cancer incidence, mortality or all-cause mortality.[/FONT]
[FONT="]Authors' conclusions[/FONT]
[FONT="]There is currently no evidence to support recommending vitamins such as alpha-tocopherol, beta-carotene or retinol, alone or in combination, to prevent lung cancer. A harmful effect was found for beta-carotene with retinol at pharmacological doses in people with risk factors for lung cancer (smoking and/or occupational exposure to asbestos). More research from larger trials and with longer follow-up is needed to analyse the effectiveness of other supplements.[/FONT]

·[FONT="]بالنسبة للأشخاص الذين لديهم عوامل خطر سواء مدخنين أو يتعرضون لخطر مهني هو الاسبستوز لا يوجد نقص في نسبة حدوث أو وفيات سرطان الرئة عند الاشخاص الذين أخذو الدواء مقارنة مع الذين أخذوا البلاسيبو و النسب تظهر أن هؤلاء الأشخاص الذين أخذوا الفيتامينات ( في هذه الدراسة [/FONT]Beta-carotine & Retinol & a-tocopherol[FONT="] ) [/FONT]

[FONT="]علمت باللون الأحمر الشيء المهم فقط و لا داعي للترجمة لأنه سهل [/FONT]
[FONT="]Main results[/FONT]
[FONT="]We identified 20 randomised trials (211,818 participants), assessing beta-carotene (12 trials), vitamin A (4 trials), vitamin C (8 trials), vitamin E (10 trials), and selenium (9 trials). Trials quality was generally high. Heterogeneity was low to moderate. Antioxidant supplements were without significant effects on gastrointestinal cancers (RR 0.94, 95% CI 0.83 to 1.06). However, there was significant heterogeneity (I2 = 54.0%, P = 0.003). The heterogeneity may have been explained by bias risk (low-bias risk trials RR 1.04, 95% CI 0.96 to 1.13 compared to high-bias risk trials RR 0.59, 95% CI 0.43 to 0.80; test of interaction P < 0.0005), and type of antioxidant supplement (beta-carotene potentially increasing and selenium potentially decreasing cancer risk). The antioxidant supplements had no significant effects on mortality in a random-effects model meta-analysis (RR 1.02, 95% CI 0.97 to 1.07, I2 = 53.5%), but significantly increased mortality in a fixed-effect model meta-analysis (RR 1.04, 95% CI 1.02 to 1.07). Beta-carotene in combination with vitamin A (RR 1.16, 95% CI 1.09 to 1.23) and vitamin E (RR 1.06, 95% CI 1.02 to 1.11) significantly increased mortality. Increased yellowing of the skin and belching were non-serious adverse effects of beta-carotene. In five trials (four with high risk of bias), selenium seemed to show significant beneficial effect on gastrointestinal cancer occurrence (RR 0.59, 95% CI 0.46 to 0.75, I2 = 0%).[/FONT]
[FONT="]Authors' conclusions[/FONT]
[FONT="]We could not find convincing evidence that antioxidant supplements prevent gastrointestinal cancers. On the contrary, antioxidant supplements seem to increase overall mortality. The potential cancer preventive effect of selenium should be tested in adequately conducted randomised trials

[FONT="]Main results[/FONT]
[FONT="]Five RCTs comparing the efficacy of four different retinoids were identified. Two studies examined the effect on CIN2 and CIN3 of retinoids N-(4-hydroxyphenyl)retinamide (fenretinide) (Follen 2001) and 9-cis-retinoic acid (aliretinoin) (Alvarez 2003) given orally and two examined the effect of all-trans-retinoic acid given topically to the cervix (Meyskens 1994; Ruffin 2004). The fifth study investigated the use of 13-cis-retinoic acid (isotretinoin) given orally in HIV positive patients with CIN1 and condyloma (Robinson 2002).
Four studies reported no significant effect of retinoids on the progression to higher grades of CIN, whilst the fifth did not report data on progression. In all studies retinoids had no significant effect on regression of CIN3. Two studies reported that retinoids were associated with regression of CIN2. One reported a greater complete regression of CIN2 over placebo, which was of borderline statistical significance, odds ratio
(OR) = 0.5 (95% confidence interval (CI) 0.25 to 1.02). The other study reported a non-significant dose-related trend towards increased rates of complete and partial regression compared with placebo. One study reported a significantly worse outcome in women receiving retinoid, OR for regression = 6.00 (95% CI 1.00 to 35.91).
In general, the retinoid medications were well tolerated.[/FONT]
[FONT="]Authors' conclusions[/FONT]
[FONT="]The retinoids studied are not effective at causing regression of CIN3 but may have some effect on CIN2[/FONT][FONT="]. The data on CIN1 is inadequate. Retinoids are not effective at preventing progression of CIN of any grade. At the doses given and duration of treatment studied, the retinoids were reasonably well-tolerated.[/FONT]

الاولى ان الدراسات عادة ما تحدد نوع محدد من السرطان و فيتامينات محددة و هذا أساسي لنجاح الدراسة لأنه من شبيه المستحيل اجراء دراسة على كل الفيتامينات و كل السرطانات يعني ما بقدر قول انو الفيتامينات تزيد او تنقص و انما أقول ان فيتامين ما يزيد أو ينقص بنسبة محددة نسبة حدوث سرطان ما بالضبط ....
الثانية أن الدراسات لم تشمل كل الفيتامينات و كذلك لم تشمل كل السرطانات ...
الثالثة أن الدراسة التي وجدتها لتبرير انه بصورة عامة هناك ميل لزيادة نسبة حدوث السرطان في بعض الحالات هي غير معتمدة لكنها من ناحية أخرى تقدم تفسير مقبول حيث ان الخلايا الورمية كما الخلايا الطبيعية تستفيد من الفيتامين أيضا للبقاء على قيد الحياة
و أخر ملاحظة و اللي هي أيضا اعتذار من جديد عن نقص ترجمة الموضوع .... لكن كان رح يتأخر الجواب اكتر من هيك بكتير
