Contraindications

Hypersensitivity to trimethadione


Warnings/Precautions

May cause severe blood dyscrasias; use with caution in patients with renal and hepatic impairment, SLE, myasthenia gravis, or intermittent porphyria; do not abruptly discontinue medication


Adverse Reactions

Central nervous system: Drowsiness, hiccups

Dermatologic: Alopecia, exfoliative dermatitis, rash

Endocrine & metabolic: Porphyria

Gastrointestinal: Anorexia, vomiting, stomach upset, abdominal pain, weight loss

Hematologic: Aplastic anemia, agranulocytosis, thrombocytopenia,

Hepatic: Hepatitis, jaundice

Neuromuscular & skeletal: Myasthenia gravis-like syndrome

Ocular: Diplopia, photophobia, hemeralopia, nystagmus, scotomata

Mechanism of Action

An oxazolidinedione with anticonvulsant sedative properties; elevates the cortical and basal seizure thresholds, and reduces the synaptic response to low frequency impulses


Usual Dosage

Oral:

Adults: Initial: 900 mg/day in 3-4 equally divided doses, increase by 300 mg/day at weekly intervals until therapeutic results or toxic symptoms appear

Dosing interval in renal impairment:

Clcr 10-50 mL/minute: Administer every 8-12 hours

Clcr <10 mL/minute: Administer every 12-24 hours

Patient Information

Blood test monitoring must be performed periodically, notify physician of persistent or severe fatigue, sore throat, fever, rash, unusual bleeding or bruising; may take with food, may cause drowsiness, impair judgment and coordination, and blurred vision; visual disturbances are normally controlled by reduction of dose


Nursing Implications

Caution patient that even minor skin rash and signs of infection or bleeding must be reported to the physician
