Adverse Reactions

1% to 10%:

Gastrointestinal: Nausea, vomiting, GI distress, GI ulcers, GI bleeding

Hematologic: Platelet inhibition

Hepatic: Hepatotoxicity

Respiratory: Bronchospasm/wheezing


Drug Interactions

Ammonium chloride, vitamin C (high dose), methionine, antacids, urinary alkalinizers, carbonic anhydrase inhibitors, corticosteroids, nizatidine, alcohol, ACE inhibitors, beta-blockers, loop diuretics, methotrexate, probenecid, sulfinpyrazine, spironolactone, sulfonylureas


Mechanism of Action

Inhibits prostaglandin synthesis, acts on the hypothalamus heat-regulating center to reduce fever; decreases pain receptor sensitivity. Other proposed mechanisms of action for salicylate anti-inflammatory action are lysosomal stabilization, kinin and leukotriene production, alteration of chemotactic factors, and inhibition of neutrophil activation. This latter mechanism may be the most significant pharmacologic action to reduce inflammation.


Pharmacodynamics/Kinetics

Absorption: From the stomach and small intestine

Serum half-life, aspirin: 15-20 minutes; metabolic pathways are saturable such that salicylates half-life is dose-dependent ranging from 3 hours at lower doses (300-600 mg), 5-6 hours (after 1 g) and 15-30 hours with higher doses; in therapeutic anti-inflammatory doses, half-lives generally range from 6-12 hours


Usual Dosage

Adults: Oral: 325-650 mg every 4 hours

Nursing Implications

Sodium content of 1 g: 6.25 mEq; less effective than an equal dose of aspirin in reducing pain or fever; patients hypersensitive to aspirin may be able to tolerate
