Use with caution in patients with impaired renal function, impaired cardiac function or seizure disorder


Adverse Reactions

1% to 10%:

Hematologic: Positive Coombs' reaction, hemolytic anemia

Renal: Interstitial nephritis

Miscellaneous: Hypersensitivity reactions, Jarisch-Herxheimer reaction


Overdosage/Toxicology

Many beta-lactam-containing antibiotics have the potential to cause neuromuscular hyperirritability or convulsive seizures

Hemodialysis may be helpful to aid in the removal of the drug from the blood, otherwise most treatment is supportive or symptom directed


Drug Interactions

Probenecid, tetracyclines, methotrexate, aminoglycosides

Mechanism of Action

Inhibits bacterial cell wall synthesis by binding to one or more of the penicillin binding proteins (PBPs); which in turn inhibits the final transpeptidation step of peptidoglycan synthesis in bacterial cell walls, thus inhibiting cell wall biosynthesis. Bacteria eventually lyse due to ongoing activity of cell wall autolytic enzymes (autolysins and murein hydrolases) while cell wall assembly is arrested.


Usual Dosage

I.M.:

<30 lb: 600,000 units in a single dose

30-60 lb: 900,000 units to 1.2 million units in a single dose

Children >60 lb and Adults: 2.4 million units in a single dose


Administration

Administer by deep I.M. injection in the upper outer quadrant of the buttock; give around-the-clock rather than 4 times/day, 3 times/day, etc, (ie, 12-6-12-6, not 9-1-5-9) to promote less variation in peak and trough serum levels; do not administer I.V., intravascularly, or intra-arterially. In infants and children, I.M. injections should be made into the midlateral muscle of the thigh.


Monitoring Parameters

Observe for signs and symptoms for anaphylaxis during first dose


Test Interactions

May interfere with urinary glucose tests using cupric sulfate (Benedict's solution, Clinitest®); may inactivate aminoglycosides in vitro; positive Coombs' [direct], increased protein
