Trace element added to TPN (total parenteral nutrition) solution to prevent manganese deficiency; orally as a dietary supplement

Contraindications

High manganese levels; patients with severe liver dysfunction or cholestasis (conjugated bilirubin >2 mg/dL) due to reduced biliary excretion


Overdosage/Toxicology

Acute poisoning due to ingestion of manganese or manganese salts is rare owing to poor absorption of manganese. The main symptoms of chronic poisoning, either from injection or usually inhalation of manganese dust or fumes in air, include extrapyramidal symptoms that can lead to progressive deterioration in the central nervous system.


Stability

Compatible with electrolytes usually present in amino acid/dextrose solution used for TPN solutions


Mechanism of Action

Cofactor in many enzyme systems, stimulates synthesis of cholesterol and fatty acids in liver, and influences mucopolysaccharide synthesis


Pharmacodynamics/Kinetics

Distribution: Concentrated in mitochondria of pituitary gland, pancreas, liver, kidney, and bone

Elimination: Mainly in bile, urinary excretion is negligible


Usual Dosage

Infants: I.V.: 2-10 mcg/kg/day usually administered in TPN solutions

Adults:

Oral: 20-50 mg/day

RDA: 2-5 mg/day

I.V.: 150-800 mcg/day usually administered in TPN solutions


Administration

Do not give I.M. or by direct I.V. injection since the acidic pH of the solution may cause tissue irritations and it is hypotonic

Periodic manganese plasma level

4-14 mg/L

Injection, as chloride: 0.1 mg/mL (10 mL)

Injection, as sulfate: 0.1 mg/mL (10 mL, 30 mL)

Tablet: 20 mg, 50 mg
