Use

Treatment of susceptible bacterial infections, mainly those caused by streptococci and staphylococci resistant to other agents

Contraindications

Minor bacterial infections or viral infections; hypersensitivity to lincomycin or any component or clindamycin


Adverse Reactions

1% to 10%: Gastrointestinal: Nausea, vomiting, diarrhea


Drug Interactions

Decreased effect with erythromycin

Increased activity/toxicity of neuromuscular blocking agents


Mechanism of Action

Lincosamide antibiotic which was isolated from a strain of Streptomyces lincolnensis; lincomycin, like clindamycin, inhibits bacterial protein synthesis by specifically binding on the 50S subunit and affecting the process of peptide chain initiation. Other macrolide antibiotics (erythromycin) also bind to the 50S subunit. Since only one molecule of antibiotic can bind to a single ribosome, the concomitant use of erythromycin and lincomycin is not recommended.


Pharmacodynamics/Kinetics

Absorption: Oral: ~20% to 30%

Serum half-life, elimination: 2-11.5 hours

Time to peak serum concentration:

Oral: 2-4 hours

I.M.: 1 hour


Usual Dosage

Children >1 month:

Oral: 30-60 mg/kg/day in divided doses every 8 hours

I.M.: 10 mg/kg every 8-12 hours

I.V.: 10-20 mg/kg/day in divided doses every 8-12 hours

Adults:

Oral: 500 mg every 6-8 hours

I.M.: 600 mg every 12-24 hours

I.V.: 600-1 g every 8-12 hours up to 8 g/day

Dosing interval in renal impairment:

Clcr 10-50 mL/minute: Administer every 6-12 hours

Clcr <10 mL/minute: Administer every 12 hours

Dosing adjustment in hepatic impairment: Reductions are indicated


Dietary Considerations

Decreased absorption with food; should be administered on empty stomach with water


Administration

Administer oral dosage form with a full glass of water to minimize esophageal ulceration; administer around-the-clock to promote less variation in peak and trough serum levels

May cause dizziness

May cause granulocytopenia; use caution with clozapine and carbamazepine
