Use

Management of mild to moderate hypertension; treatment of edema in congestive heart failure and nephrotic syndrome, and cirrhosis of the liver accompanied by edema and/or ascites

Contraindications

Anuria, hyperkalemia, renal or hepatic failure, hypersensitivity to hydrochlorothiazide, spironolactone, or any component


Adverse Reactions

1% to 10%:

Dermatology: Rash

Endocrine & metabolic: Hyperkalemia, gynecomastia, hyperchloremic metabolic acidosis (in decompensated hepatic cirrhosis), dehydration, hyponatremia

Gastrointestinal: Anorexia, nausea, vomiting, diarrhea


Usual Dosage

Oral:

Adults: 1-8 tablets in 1-2 divided doses


Dietary Considerations

Food with high potassium content, natural licorice, and salt substitutes should be avoided


Cardiovascular Considerations

Combination therapy for the treatment of hypertension should be individualized for each patient. Potential advantages for hydrochlorothiazide and spironolactone combination therapy may include improved compliance and synergistic reductions in blood pressure with an accompanied reduction in side effects (eg, hypokalemia). Thiazide and spironolactone also are effective in the management of heart failure.
