Contraindications

Undiagnosed vaginal bleeding; hypersensitivity to estrogens or any component; thrombophlebitis, liver disease, known or suspected pregnancy, carcinoma of the breast, estrogen dependent tumor, thromboembolic disorders


Warnings/Precautions

Use with caution in patients with a history of hypercalcemia, cardiac disease, and gallbladder disease. The addition of progestins may attenuate estrogen's effects on raising HDL and lowering LDL cholesterol. May increase blood pressure and serum triglycerides (in patients with familial dyslipidemias). Use caution in patients with hepatic disease or renal dysfunction; may increase risk of venous thromboembolism. Estrogens have been reported to increase the risk of endometrial carcinoma and may increase the risk of breast cancer. Safety and efficacy in children have not been established. Do not use estrogens during pregnancy.

Cardiovascular: Palpitation (21%), peripheral edema (10%)

Endocrine & metabolic: Breast pain (29%), menorrhagia (14%)

Central nervous system; Headache (68%), insomnia (42%), paresthesia (33%), nervousness (28%), depression (28%), pain (11%), dizziness (11%)

Gastrointestinal: Abdominal pain (28%), flatulence (29%), nausea (18%), dyspepsia (10%)

Musculoskeletal: Myalgia (28%), arthralgia (25%), back pain (14%)

Miscellaneous: Weakness (33%), infection (14%)

1% to 10%:

Gastrointestinal: Vomiting (7%), constipation (6%), diarrhea (6%)

Central nervous system: Hypertonia (6%), fever (1%)

Musculoskeletal: Leg cramps (10%)

Respiratory: Pharyngitis (8%), rhinitis (8%), cough (6%)

Additional adverse reactions associated with estrogen therapy include: Increase in blood pressure, hypercalcemia, thromboembolic disorder, myocardial infarction, hypertension, anxiety, stroke, chorea chloasma, melasma, rash, erythema multiforme, erythema nodosum, alopecia, hirsutism, breast tumors, amenorrhea, alterations in frequency and flow of menses, changes in cervical secretions, pancreatitis, decreased glucose tolerance, weight gain, weight loss, increased triglycerides and LDL, GI distress, cholestatic jaundice, aggravation of porphyria, breast tenderness, breast enlargement, changes in libido, intolerance to contact lenses, changes in corneal curvature, increased susceptibility to Candida infection


Overdosage/Toxicology

Symptoms of overdose include fluid retention, jaundice, and thrombophlebitis. Toxicity is unlikely following single exposures of excessive doses, any treatment following emesis and charcoal administration should be supportive and symptomatic.

Patient Information

Follow prescribed schedule and dose. Periodic gynecologic exam and breast exams are important with long-term use. Consult prescriber for specific dietary recommendations. You may experience nausea or vomiting (small frequent meals may help); dizziness or mental depression (use caution when driving); photosensitivity (use sunscreen, wear protective clothing and eyewear, and avoid direct sunlight); rash, loss of scalp hair (reversible); enlargement/tenderness of breasts (both male and female); increased (female)/decreased (male) libido; or headache (use of mild analgesic may help). Report swelling of extremities or unusual weight gain; chest pain or palpitations; sudden acute pain, warmth, or weakness in legs or calves; shortness of breath; severe headache or vomiting; or unusual vaginal bleeding, amenorrhea, or alterations in frequency and flow of menses.
