The liver , the largest organ in the body , is often the target organ for chemically in duced injuries

Chemicals produce a wide variety of clinical and pathological hepatic injury.
Biochemical markers (i.e. alanine transferase, alkaline phosphatase and bilirubin) are often used to indicate liver damage.
Liver injury is defined as rise in either
(a) ALT level more than three times of upper limit of normal (ULN),
b) ALP level more than twice ULN ,
c) total bilirubin level more than twice ULN when associated with i
Liver damage is further characterized into hepatocellular (predominantly initial Alanine transferase elevation) and cholestatic (initial alkaline phosphatase rise) types.

However they are not mutually exclusive and mixed type of injuries are often encountered.
Specific histo-pathological patterns of liver injury from drug induced damage are discussed below

Zonal Necrosis

This is the most common type of drug induced liver cell necrosis where the injury is largely confined to a particular zone of the liver lobule. It may manifest as very high level of ALT and severe disturbance of liver function leading to acute liver failure

Causes
:
Paracetamol (Acetaminophen,Tylenol), carbon tetrachloride


Hepatitis

In this pattern hepatocellular necrosis is associated with infiltration of inflammatory cells.
There can be three types of drug induced hepatitis.
(A) viral hepatitis type picture is the commonest, where histological features are similar to acute viral hepatitis.
(B) in the focal or non specific hepatitis scattered foci of cell necrosis may accompany lymphocytic infiltrate.
(C) chronic hepatitis type is very similar to autoimmune hepatitis clinically, serologically as well as histologically

Causes
:

Viral hepatitis like: Halothane, Isoniazid, Phenytoin
(b) Focal hepatitis: Aspirin
(c) Chronic hepatitis: Methyldopa, Diclofenac

Cholestasis
Liver injury leads to impairment of bile flow and clinical picture is predominated by itching and jaundice. Histology may show inflammation (cholestatic hepatitis) or it can be bland without any parenchymal inflammation. In rare occasions it can produce features similar to primary biliary cirrhosis due to progressive destruction of small bile ducts (Vanishing duct syndrome
).
Causes:
(
a) Bland: Oral contraceptive pills, anabolic steroid, Androgens
(b) Inflammatory: Allopurinol, Co-amoxiclav, Carbamazepine
(c) Ductal: Chlorpromazine, flucloxacillin


Steatosis
[align=left]Hepatotoxicity may manifest as triglyceride accumulation which leads to either small droplet (microvesicular) or large droplet (macrovesicular) fatty liver. There is a separate type of steatosis where phospholipid accumulation leads to a pattern similar to the diseases with inherited phospholipid metabolism defects (e.g. Tay-Sachs disease[/align]

a) Microvesicular: Aspirin (Reye's syndrome), Ketoprofen, Tetracycline
(b) Macrovesicular: Acetamenophen, methotrexate
(c) Phospholipidosis: Amiodarone, Total parenteral nutrition


Granuloma
Drug induced hepatic granulomas are usually associated with granulomas in other tissues and patients typically have features of systemic vasculitis and hypersensitivity. More than 50 drugs have been implicated
.
Causes:
Allopurinol, Phenytoin, Isoniazid, Quinine, Penicillin, Quinidine


Vascular lesions
They result from injury to the vascular endothelium

Causes:

Venoocclusive disease: Chemotherapeutic agents, bush tea
Peliosis hepatis: anabolic steroid
Hepatic vein thrombosis: Oral contraceptives

Neoplasm
Neoplasms have been described with prolonged exposure to some medications or toxins. Hepatocellular carcinoma, angiosarcoma and liver adenomas are the ones usually reported
.
Causes
:
Vinyl chloride, Combined oral contraceptive pill,Anabolic steroid, Arsenic, Thorotrast

    جزيل الشكر دكتورة سارة

    برأيي لا , ما لازم نعطيه بصورة عامة لأنو ما تم تحديد درجة الأذية الكبدية ( لها أربع درجات للأذية حسب المكان الذي وصله التنخر )
    و بحال سوء وظيفة الكبد فلا نعلم ما هي الجرعة اللي ممكن تكون غير سامة و اللي ممكن تكون دون الجرعة الفعالة الدنيا

    و من ثم هناك أدوية أخرى ممكن اعطاءها و حتكون ساف

    يجب علينا أولاً أن نعرف مدى الأذية الكبدية؟وثانياً أن نعرف مقدار الجرعة المعطاة من الباراسيتامول؟ فسمية الباراسيتامول الكبدية تبدأ عند جرعة 4 غ
    برأي :لانعطيه البارسيتامول ونستعيض عن بمشتقات حمض الساليسيليك (أسبرين-صوديوم ساليسيلات..) أو مشتقات أقوى منها مثل مشتقا البيرازولون(أمينو فينازون..)حسب التاريخ المرضي

    أولاً شكراً على النظرة العامة على أمراض الكبد و فعلاً موضوع مفيد بشكله المختصر

    بالنسبة للحالة : غلط كبير إعطاء المريض باراسيتامول لأن بالبارسيتامول سيمر على الكبد للإستقلاب و متل ماقالوا السباب هذا لايجوز أما الصداع بيجب البحث عن منشأ الصداع و على أساس المنشأ يوصف الدواء فأحتمال أن يكون شقيقة أو صداع التوتري أو العنقودي .....

    لو استخدم لوحده
    لكن ف ف السوق المصرى دوا فيه combination بين ال
    paracetamol و methionine وده ساف جدا ف حاله ال liver disfunction , وبالتالى يمكن استخدامه
    واسم الpreparation ده HEPAMOL

    paracetamol possesses analgesic & antipyretic properties but has weak anti-inflammatory activity
    . DL-methionine is racemic mixture of the essential amino acid L-methionin & its isomer
    methionine enhance hepatic synthesis of glutathioe which is used to activate the toxic quinone metabolite of paracetamol by conjugation


    :HEPAMOL is used as
    Analgesic : as in case of headache , migraine ,rheumatic & muscular pain .toothache & dysmenorrhoae
    Antipyretic : relief symptom of comm cold & influenza

    PARACETAMOL



    Excessive use of paracetamol can damage multiple organs, especially the liver and kidney. In both organs, toxicity from paracetamol is not from the drug itself but from one of its metabolites, N-acetyl-p-benzoquinoneimine (NAPQI).
    In the liver, the cytochrome P450 enzymes CYP2E1 and CYP3A4 are primarily responsible for the conversion of paracetamol to NAPQI.
    In the kidney, cyclooxygenases are the principal route by which paracetamol is converted to NAPQI.
    Paracetamol overdose leads to the accumulation of NAPQI, which undergoes conjugation with glutathione. Conjugation depletes glutathione, a natural antioxidant. This in combination with direct cellular injury by NAPQI, leads to cell damage and death.



    Signs and symptoms of paracetamol toxicity may initially be absent or vague. Untreated, overdose can lead to liver failure and death within days.

    Treatment is aimed at removing the paracetamol from the body and replacing glutathione. Activated charcoal can be used to decrease absorption of paracetamol if the patient presents for treatment soon after the overdose. While the antidote, acetylcysteine, (also called N-acetylcysteine or NAC) acts as a precursor for glutathione helping the body regenerate enough to prevent damage to the liver, a liver transplant is often required if damage to the liver becomes severe.
